WHO DO YOU OFFER YOUR SERVICES TO?

When a person has been diagnosed with a life limiting illness and the goals of care are for relief of symptoms and quality of life, she/he can be referred to the Palliative Care Team.

WHAT SERVICES DO YOU OFFER, AS YOU LOOK THROUGH A HEALTH LENS?

The Palliative Care Nurse works with the patient's primary health care provider to manage the patient's care and symptoms. The nurse can provide home visits and assess the needs of the patient and family. She/he can provide information about end of life care, provide access to Social Work support, teach families about medication, access support from a Palliative Care Physician and offer various options to maximize quality of life. The Palliative Care Program offers after hours telephone support on evenings, nights and weekends once a patient is registered with the Program. The team can support patients in their home for end of life care.
