Rat River Health Service Directory
We are very proud to launch our 1st issue of the HEALTH NETWORKING TOOL for services in the RM of De Salaberry and the Village of St-Pierre-Jolys.
The goal is to provide opportunities for collaboration and to help us all see that our communities have a lot of resources to complement the traditional health care services. Sometimes we don’t even realize the important role each plays in our health.

By working together, we can be more successful and realize that as individuals and as health care providers, we are not alone in this great health care responsibility.
Thank you all for caring!

Quick Facts
To change the underlying risk factors that lead to chronic diseases and poor health, it is important to reinforce education on healthy behaviors and to support social services needs between clinic visits.
Tailoring interventions to community needs and resources is important, as is building a trusting relationship between clinical and community partners and the community members served.
About this Tool
TO CONSUMERS :
This tool is a collection of LOCAL government and non-government (non-profit organizations, businesses, etc.) that help keep us healthy. It can also help identify gaps to fill i.e. what services are lacking in our area that we could help create?
To providers :
Collaborations between clinical, community and public health organizations offer a win-win scenario and have been shown to reduce and prevent disease in communities and provide additional support to help patients maintain healthy behaviors.
The Rat River Health Council 2008 Inc. was created by the community, for the community in 1994. Councillors from the RM of De Salaberry and the Village of St-Pierre-Jolys sit on its Board as well as community members representing community organizations or the community at large.
